Asthma and Allergic Rhinitis Questionnaire

The 'Asthma and Allergic Rhinitis Questionnaire' is a patient self-assessment tool that you can use in discussions with your doctor, nurse or pharmacist.

Why should you complete the questionnaire?

This is a simple questionnaire that will give your doctor, nurse or pharmacist information about how well your asthma and allergic rhinitis are managed.  Your doctor, nurse or pharmacist may ask you further questions and this may serve to help you manage your condition better.

Who created the questionnaire?

The questionnaire was created by 3 organisations:

• Allergy UK is the country's leading medical charity dealing with allergy.
• Asthma UK is the charity dedicated to improving the health and well-being of the 5.4 million people in the UK whose lives are affected by asthma.
• Education for Health is the UK's leading education charity for health professionals working with patients with long-term conditions.

The campaign was initiated and funded by MSD.
